• If your state is not listed please use the Multi-Jurisdictional form.
• Florida: please send your current year Resale Cert the State issued you.
• If you are not Sales Tax Exempt: please make it aware when sending your application, you will be charged your state's sales tax rate on all purchases.

Any returned check, NSF or stop payment, will be subject to a $50.00 administrative fee or 10% of the check amount whichever is greater.
